day.GENERAL FUNCTIONProvide support to a group of Blood Pressure
Self-Monitoring program participants as theyadopt the habit of
self-monitoring and tracking their blood pressure in order to
achievebetter blood pressure management.QUALIFICATIONS:

Seekers with chronic diseasesAbility to provide positive feedback
and build relationshipsWorking knowledge of high blood pressure and
the wants, needs, and interests ofadults with high blood
pressureConsistently positive, patient, and non-judgmental attitude
toward participantsAbility to follow strict program delivery
guidelines with consistencyAbility to effectively utilize computer
software technology to manage programactivities and track
participant progress, if assigned REDCap data entry
dutiesPreferred: strong facilitation skillsPRINCIPAL DUTIES:
Assist with program promotions and
participant recruitment activities as directed byProgram
ManagerEnroll new participants in the Blood Pressure
Self-Monitoring programSend weekly reminder and support messages to
caseload of participantsProvide 10-minute consultations to program
participants during established OfficeHoursMeasure and record
participant blood pressures, modeling correct technique,
duringEnrollment, Office Hours, and at Nutrition Education
SeminarsUtilize REDCap web-based software to track participant
information, Office Hours andNutrition Education Seminar
attendance, and blood pressure readings in an accurateand timely
manner as assigned by program managerMaintain participant
confidentiality by managing program records and personalhealth
information according to best practicesAccept payment for class and
provide receipts to participants whenconducting class at a non-Y
locationFacilitate 60-minute Nutrition Education Seminars as
neededPHYSICAL REQUIREMENTS:The physical demands described here are
