Endoscopy

Podiatry

General

Orthopedics

Urology

Adult patients

Spinals, blocks, and epidurals required

Case logs required within the past 2 years, including proof of epidurals, spinals, and blocks

Operating rooms: 4

Required Qualifications

CRNA

Certifications: BLS, ACLS, PALS (AHA required)

Credentialing

Estimated credentialing timeframe: 60 to 90 days

Temporary privileges may be available with a clear background and malpractice history and an active unrestricted license
