Registered Nurse (RN) | Hospice Location: Fort Wayne, IN Agency: Cynet Health Pay: $2,106 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/2/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Hospice RN in Fort Wayne, Indiana, 46825! Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one (1) year experience as an RN, either in acute care hospital, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the state of employment. Certifications: BLS (American Heart Association). Must-Have: Current automobile insurance and valid driver's license. Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family. The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN assures the highest quality of care is provided and documented in accordance with the Plan of Care. The RN performs the initial and ongoing assessment of the patient. The RN helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with other members of the interdisciplinary group. The RN works closely with other members of the interdisciplinary group to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services including bereavement support to the patient and family to achieve the highest quality of care. The RN reports any change in the patient's condition to both the attending physician and hospice physician. The RN maintains an accurate and updated medical record, including all interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other information pertinent to the care of the patient. The RN practices infection control measures and standard precautions. The RN monitors scheduling of all staff. The RN participates in the Quality Assessment and Performance Improvement Program. The RN may be requested to participate in the agency on-call schedule. The RN consistently promotes the company’s core values. This is a safety-sensitive position. The RN completes required compliance annual training. The RN performs other activities as assigned. The RN attends staff meetings. The RN maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN makes routine supervisory visits of, and with, all other team members at least monthly. The RN performs ongoing review and follow-up on assigned patient clinical records to assure quality services are provided, standards of practice are followed, and compliance with policies, procedures, and regulatory requirements is met and maintained. The RN maintains close contact with the patient’s family and/or significant others to provide information, support, and continuity of care. The RN maintains primary control and professional management of each patient and acts as the primary liaison between the physician, patient/family, and hospice team to be fully aware and abreast of all clinical issues, staffing, and scheduling concerns. The RN is responsible for the education of the patient, family, or other caregivers in relation to needs identified on the Plan of Care, including but not limited to medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient. 5 Benefits of Travel Nursing Advocating for patients becomes more nuanced when dealing with diverse populations.
