About the Position Job Title: Integrated Case Manager Profession:

Hours per Shift: 8:00 AM - 4:30 PM, rotating weekends Experience:
Minimum 5 years of Acute Inpatient Case Management experience;
Recent (within the last two years) acute inpatient case management
experience. License: Current valid RN licensure or LMSW with
current valid licensure. Certifications: BLS certification through
AHA. Must-Have: - Strong computer skills. - Epic EMR experience. -
Acute discharge planning experience. Description: The Integrated
Case Manager for Population Health is an interdependent member of
the patient-centered care team responsible for the collaborative
practice of assessment, planning, facilitation, care coordination,
evaluation, and advocacy for options and services. This role aims
to meet an individual's and family's comprehensive health care
needs through effective communication and available resources. The
focus is on promoting patient safety, quality of care, and
cost-effective outcomes. The manager addresses the needs of
patients who have experienced a critical event or diagnosis that
requires complex management strategies. This involves the extensive
use of resources to optimize health outcomes along the care
continuum. The Integrated Case Manager provides services to
patients from various settings, including ambulatory and inpatient.
Key responsibilities include: Conducting comprehensive assessments
of patients and family or caregiver needs. Developing personalized
patient-centered care plans aimed at optimizing care experiences.
Engaging patients and their families as part of the care team
through advocacy and health education. Utilizing professional
judgment and critical thinking to assist patients in overcoming
barriers to their goals. Providing counseling related to treatment
decisions and end-of-life issues. Ensuring patients transition
safely between care settings. Advocating for appropriate delivery
of services within the patient's health plan benefit structure.
Collaborating with treatment team members to co-manage patients
with complex needs. Facilitating care conferences to promote
interdependent collaboration. Evaluating patients’ responses to
care plans and making modifications as necessary. Participating in
process improvement activities designed to reduce risk through data
collection and follow-up. Facilitating interventions in cases
involving abuse and neglect. Supporting department-based goals that
contribute to organizational success. Performing other duties as
assigned. Candidates must have excellent verbal communication and
documentation skills. Customer service and interpersonal skills are
essential for interacting with internal and external customers.
Strong problem-solving, analytical, and decision-making skills are
required. Knowledge of preventive service guidelines, clinical
practice guidelines, and case management principles is preferred.
Understanding social determinants of health and their influence on
patient wellbeing is vital. Strong organizational, planning, and
implementation skills are needed to handle multiple complex patient
needs simultaneously. Compassion and the ability to advocate for
patients and their families are critical for this position.
