providers coordinate patients’ care among a team of specialists –
podiatric physicians and surgeons, advanced practice providers,
care navigators, pharmacists, dieticians, and social workers for
integrated treatment that addresses patients’ immediate and

value-based care. About the Endocrinologist Role Upperline Health
is seeking a well-trained and compassionate Endocrinologist (MD/DO)
to join our growing specialty care team. This position provides a
unique opportunity to practice within a multidisciplinary care
model designed to proactively manage diabetes and endocrine
disorders, with a strong focus on preventing complications such as
lower-extremity wounds and amputations. This position is a
clinic-based setting with the ability to provide both consultative
and longitudinal care, supported by care coordination teams and
