Description: Zepf Center has been serving the Lucas County
community for over 50 years. We are the leading provider of
behavioral health and substance use disorder services in Northwest
Ohio. Services include adult and child psychiatric, substance
abuse, case management, residential, Crisis Care, and therapy
programs, as well as career development and wellness services. Zepf
Center also offers primary care medical services to our patients to
contribute to their continuum of care. Zepf Center is a
trauma-informed agency and environment for both patients and staff.
Hours: Monday - Friday; 8:30am - 4:30pm *Salary Range: $112K -

Summary: The purpose of this position is to provide comprehensive,
patient-centered primary care services as a Family Nurse
Practitioner (FNP) within the integrated healthcare setting of Zepf
Center. The FNP will deliver advanced nursing and clinical
services, including health assessments, diagnosis, treatment, and
follow-up care for a diverse patient population across the
lifespan. The FNP will collaborate with a multidisciplinary team to
promote wellness, prevent illness, manage chronic conditions, and
ensure continuity of care. Responsibilities include providing
advocacy for patients, coordinating referrals to specialty
providers, managing physical health symptoms, prescribing and
evaluating medications, and delivering high-quality clinical and
nursing services in alignment with organizational goals and patient
care standards. This is a full-time position based in a Primary
Care clinic setting, requiring a proactive, compassionate provider
who is committed to integrated, whole-person care. Essential Duties
and Responsibilities: Practices in accordance with the Standard
Care Arrangement and maintains all required licensure and
continuing education necessary for Advanced Practice Nursing (APN)
in the State of Ohio. Collaborates with the designated
Collaborating Physician, as required by Ohio State Law and the
Standard Care Arrangement, to discuss and resolve treatment-related
concerns. Conducts medical evaluations, including physical
examinations, diagnoses, and treatment planning for clients;
monitors the effectiveness of interventions and adjusts care as
needed. Prescribes and manages medications in accordance with the
scope of practice for APNs and the Ohio Administrative Code Chapter
4723-9; monitors for efficacy, side effects, and patient adherence.
Performs approved treatment procedures within credentialed scope.
Facilitates hospital admissions when clinically indicated and
provides consultation regarding hospitalization for Zepf clients as
needed. Responds promptly and appropriately to clinical
emergencies. Maintains accurate, timely, and compliant
documentation of clinical services, including assessments, progress
notes, treatment plans, medication orders, and other required
primary care documentation. Actively contributes to the Zepf Center
Medical Staff, collaborating with team members to address clinical
concerns and ensure quality of care. Participates in Medical
Services Team meetings, organizational development efforts, and
other collaborative initiatives as approved by the supervisor.
Adheres to all Zepf Center policies and procedures, with a strong
commitment to maintaining client confidentiality and compliance
with applicable laws. Provides education to clients and, when
appropriate, to family members or community service providers
regarding diagnoses, treatment options, and health management.
Stays current with trends, best practices, and developments in
clinical practice; completes all continuing education and training
required for licensure and organizational compliance. Performs
other duties as assigned by leadership. Specific/Individual

Position Qualifications: Must be licensed to practice nursing and
have met the qualifications as a Family Nurse Practitioner (FNP)
through a certification from certifying boards of either American
Nurses Credentialing Center (ANCC) or the American Academy of Nurse
Practitioners Certification Program (AANP). Must have prescriptive
authority with a valid DEA Registration Must have at least two
years relevant professional experience Must be able to provide
cardiopulmonary resuscitation Must be proficient and accurate in
computer use and able to handle a high usage of the computer
