Zepf Center, a leading non-profit organization in the Toledo area, is seeking a compassionate and skilled Family Nurse Practitioner to join our healthcare team. This role involves providing comprehensive healthcare services and behavioral health support to individuals and families, helping to foster healthier communities through high-quality medical care and personalized patient interactions. Conduct comprehensive patient assessments, diagnosing and treating various health issues. Develop and manage effective treatment plans in collaboration with healthcare professionals and patients. Prescribe medication and monitor its effectiveness, adjusting treatment as necessary. Provide counseling and education to patients and their families on health care issues, prevention, and treatment plans. Contribute to the multidisciplinary team with feedback and suggestions for improving patient care and outcomes. Maintain accurate and detailed medical records and documentation. Participate in ongoing education to stay abreast of innovations and practices in family and behavioral health. Proficient in clinical skills with the ability to conduct thorough health assessments and develop effective treatment plans. Strong interpersonal and communication skills, comfortable working with diverse patient populations. Ability to work collaboratively in a multidisciplinary health care team. Knowledge of behavioral health issues and their treatment options. Ability to handle a high workload and prioritize tasks effectively. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Family Nurse Practitioner (FNP-C) through the American Nurses Credentialing Center (ANCC) or equivalent. Valid state license to practice as a Nurse Practitioner. DEA certification. BLS/ACLS certification.
