Job Description:

The RN Program Coordinator is the primary point person for ensuring the collaborative care of patients from the time of hospital discharge through all follow ups. They work closely with hospital and other PPG staff to ensure smooth transition of care including testing and clinic visits. -

Gather and review eligible patient's self-reported health information.
Update the eligible patient's medical, social and family history.
Review the patient's Health Risk Assessment to identify potential barriers, risk factors, opportunities for education and gaps in recommended preventive services.
Triage phone calls; coordinate care for patient with provider and other service lines/specialties.
Furnish personalized health advice to the patient and provide referrals as appropriate for recommended testing or preventive counseling.
Update the Electronic Health Record to reflect the patient's most current information.
Establish and maintain positive working relationships with all key customers (physicians, office staff, patients, families, and interdisciplinary team).
Coordinate patient's procedures and possible hospital admissions. -
Participate in patient's clinical appointments, assisting in care of patient.
Complete special projects, as assigned by leadership team.
Graduation from accredited Nursing Program
Working knowledge of office based regulatory requirements. -
Flexible with hours to meet the needs of department.
The Coordinator must be able to work through the office and hospital based electronic health records. -
Demonstrated ability to communicate with medical and administrative staff and patients, excellent understanding levels of care. -
Bachelor's of Science in Nursing -
Skills: EPIC experience. - CARDIAC or CARDIOLOGY experience preferred
Frequent Sitting and computer use. -
