Job Description:

The Physician Team Leader will be treating patients in their homes or assisted living facility with an assigned Certified Medical Assistant. The Physician Team Leader will oversee 3-4 advance practice providers, and acts as a resource impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team. We are seeking Physicians who desire the opportunity and experience of delivering quality and compassionate healthcare - within proven care models to elderly individuals and those with complex medical issues.

Required skills and experience:

--- 3 years Experience in Geriatrics, Home Care or Urgent Care
--- Experience in treating chronic illnesses
--- Active/unrestricted medical license and DEA
--- Board Certified or Board Eligible
--- EMR experience
--- Must have the ability to lead and partner with NPs and support them with high risk patient cases.
--- Comfortable treating patients in their homes and traveling out in the field with an assigned MA, company care provided.
