you'll do We will look to you to oversee the admission and referral
process of hospice patients, including tracking, assessing, and
admitting patients into the hospice program and determining their
eligibility. You'll know the status of each hospice referral and
will maintain on-going communication with the patient, their
family, and the referral source. You will also be responsible for
tracking and follow-up for discharged patients and non-eligible
patients for future eligibility. What it takesCurrent Ohio RN
nursing license Certification as a Hospice and Palliative Nurse
preferredThree years experience in a Medicare-certified hospice
agency preferredReliable vehicle (you'll use your own car), valid
driver's license, insurance and acceptable driving record Ohio
