ProMedica Flower Hospital is located in Sylvania OH, a suburb of

tournament. -The Flower Hospital campus is a 311-bed facility that
houses the Hickman Cancer Center, ER Trauma Center, and is the
regional leader for inpatient adult psychiatric services. -The
campus includes, the Goerlich Center, which provides residence for
patients with Alzheimer's and Dementia, along with the Ebeid Center
our hospice facility.Position Summary:The clinical -dietitian
collaborates with members of the health care team to provide
medical nutrition therapy. - Identifies -patients at nutritional
risk based on hospital policy. - Assesses nutritional needs and
identifies -nutrient requirements according to -age and disease
specific needs of patients. - Develops and implements the nutrition
care plan for all assigned patients. - Fosters continuity of care
among all phases across the continuum of care. -Continuously
evaluates and monitors the effectiveness and outcomes of medical
nutrition therapy interventions. - Provides nutrition education to
patients and family members on appropriate food selections and
eating behaviors to accomplish -patient -goals. -The clinical
dietitian also participates in performance improvement activities.
- Acts as a resource for dietetic technicians -and mentors
dietetics students. - Actively assesses, evaluates and maintains
professional development and competencies.Accountabilities:
- Performs age-specific nutritional assessments and develops
age-specific medical nutrition therapy plans for all age
groups.
- Obtains accurate, complete and timely nutritional history from
patient and/or family using appropriate interviewing
techniques.
- Reviews the medical record thoroughly, accurately noting and
interpreting pertinent information in accordance with departmental
and professional standards.
- Correctly identifies patients at nutritional risk in accordance
with department policy.
- Assesses patients' learning needs, barriers to learning,
preferred learning styles and educational level and provides
nutrition education accordingly.
- Assesses nutritional needs and identifies nutrient requirements
according to age and disease specific needs of patients.
- Develops thorough, appropriate medical nutrition therapy plans
and makes interventions and/or recommendations in a timely and
effective manner.
- Coordinates patient's medical nutrition therapy with other care
team members.
- Documents individualized nutritional problems, goals, outcomes
and plan of care in medical record accurately, completely, and
concisely in accordance with hospital documentation policies.
- Continuously evaluates and monitors the effectiveness and
outcomes of medical nutrition therapy interventions.
- Plan and implements group cases or presentations as assigned in
accordance with professional and departmental standards.
- Develops and updates educational materials as needed or
assigned in accordance with professional and departmental
standards.
- Participates in performance improvement activities as
assigned.
- Assists with menu planning and revision.
- Participates as an active member of hospital committees;
represents department interests and contributes toward hospital
goals.Required Qualifications:
- Must be licensed -dietitian with the Ohio Medical Board.
- Bachelor's degree in dietetics or related field.
- Registered dietitian with the Commission on Dietetic
Registration of the Academy of Nutrition and Dietetic.
- Must be able to move about hospital and between
workstations.
- Must be able to understand directions, communicate and respond
to inquiries.
- Effective interpersonal skills.
- Must be able to use the hospital computer system to input and
