Job Description University of Toledo Physicians' mission is to

specialties from the most complex diagnoses and treatments to
primary care for the entire family. The primary site of inpatient
care services is at the University of Toledo Medical Center, but
many of our physicians--- practice at hospitals and medical offices
throughout the region. University of Toledo Physicians offers

tuition waiver at UT. Position Summary: Functions within the
collaborative agreement arranged between the Advanced Practice
Provider (APP) and physician. The APPs work autonomously and in
collaboration with the Hospitalists to diagnose, treat and manage
individuals or population of patients. APPs can serve as health
care resource providers, patient advocates, and interdisciplinary
consultants with the Health Care Team (HCT). Duties: Demonstrates
clinical competency and accountability in care for patients as
identified in unit---s Scope of Care. Assesses the physical and
psychosocial status of patients by means of interview, health
history, physical examinations, and diagnostic studies; Recognizes
deviations from normal in the assessment and coordinates care with
physician. Writes prescriptions; initiates request and/or perform
routine laboratory, radiologic and diagnostic studies as indicated.
Identify normal and abnormal findings commonly seen in laboratory
studies. Evaluates and monitor the patient---s response to health
and disease and provide education and counseling as appropriate;
Initiates appropriate actions to facilitate the implementation of
therapeutic plans that are consistent with the continuing
healthcare needs of the patient. Determines eligibility of patients
for entry into clinical trials. Requests written consultation from
physicians and other healthcare professionals to ensure appropriate
and quality patient care. Demonstrates competence of the practice
outlines within the SCA/Supervisory Agreement using methods of
quality care reviews, periodic peer review, and clinical outcomes
evaluation; Files agreement with the sponsoring physician, the
Medical Staff Office, and reviews and renews periodically. If
requirement for prescriptive authority are met, add appropriate
drugs from formulary to the plan as appropriate. Maintains
competency in performing clinical practice for patients in stages
of the life cycle appropriate to specialty. Values ethic and
cultural differences in the treatment and planning of care.
Maintains provision of preventative health services (e.g.
screening, risks assessment, immunizations). Conducts or
participates in daily multidisciplinary rounds and ensures
appropriate and quality care for assigned patients. Precepts
students. Initiates/communicates discharge planning needs to
patients/social workers in a timely manner. NP Requirements:
Graduate of an accredited Nurse Practitioner program. Master---s
degree in nursing. Current licensure by the State Medical Board of
Ohio as a Nurse Practitioner / Advanced Practice Nurse National
certification as a Nurse Practitioner. Has a Certificate of
Authority (COA) from the Ohio Board of Nursing to practice as an
Advanced Practice Nurse. PA Requirements: Graduate of a formal
Physician Assistant academic program that has been approved by the
Accreditation Review Commission on Education for the Physician
Assistant (ARC-PHYSICIAN ASSISTANT). Current Certification by the
National Commission on Certification of Physician Assistants.
Current licensure by the State Medical Board of Ohio as a Physician
Assistant with prescriptive authority. General Requirements: BLS
and ACLS certified. Possesses DEA controlled substance
registration. Possesses NPI (Nation Provider Identifier) from CMS
as rendering provider and/or Medicare PINs and UPINs or their
Medicaid provider numbers. Excellent written and verbal
communication skills and problem solving ability required. Able to
document care legibly in hand written text. Able to navigate
through the EMR and record required data accurately and
efficiently. Able to communicate and collaborate with team members
including but not limited to physicians, nurses, managers, other
APPs and other team members. Ability to identify when supervision
is necessary. Demonstration of professional behavior at all times.
