Thank you for considering a career at Mercy HealthSummary:Functioning as a member of the Care Delivery Team, the Clinical Nurse Associate/ RN New Graduate works under the direction of an RN Clinical Nurse II (CNII) or RN Preceptor to provide direct, quality nursing care for patients and performs duties as assigned. Considered an entry level professional nurse who has not yet passed the National Council Licensure Examination (NCLEX). The CN Associate/RN Associate must progress to the RN or Clinical Nurse II level in accordance with the State Board of Nursing licensing requirements for licensure.Essential FunctionsNursing Care:* Begins to utilize the nursing process to assess overt and covert physical, psychological, and sociocultural needs of all patients.* Administers medications skillfully and safely, according to policy and standard of care, under the guidance of the CNII or Preceptor.* Performs nursing/medical treatments and diagnostic tests and procedures according to policy/procedure with supervision and guidance from CN II/Preceptor which may include, but not limited to:* Taking and recording vital signs and weights.* Collecting, labeling, and sending specimens to Lab.* Observing, measuring, and recording intake of P.O. fluids and tube feedings.* Inserting and discontinuing intermittent and continuous urinary catheters with supervision or assistant of a preceptor.* Observing, measuring, and recording output of urine, drainage, vomitus, stools, suction canisters, and chest tubes.* Inserting and discontinuing peripheral IV's with supervision or assistance of a preceptor.* Performing peripheral phlebotomy with supervision or assistance of a preceptor* Performing oral suctioning, chest PT, turn/cough/deep breathing.* Performing sterile and non-sterile dressing changes and wound care.* Performing 12 lead ECGs.* Assisting with application of restraints to assure patient safety.* Monitoring for patient safety.* Application of elastic bandages, splints, anti-embolitic stockings.* Independently transport stable patients to testing areas.* Contributing to and modifying the plan of care on an ongoing basis to achieve expected outcomes including patient and family education.* Communicating with appropriate in-house resources to assist in planning patient care.* Planning and organizing work to complete patient care assignment, allows time for in-services, unit meetings, meals, etc.* Maintaining a clean and orderly patient environment, removing soiled linen, and disposing of waste, and so forth, as necessary. Follows the body substance protocol in caring for all patients.* Seeking to identify and meet customer needs and expectations.* Determining the extent to which the goals of care have been achieved and reported to CNII or Preceptor.* Evaluating the patient and/or significant others' understanding and compliance with prescribed therapy and reports to CNII or Preceptor.* Assisting in evaluating level of care and data (e.g. vital signs, I & O, weights, neuro checks) for appropriateness and accuracy.* Being aware of and in compliance with hospital, nursing, and unit specific policies, procedures, and standards.* Modeling standard professional behavior to all personnel.* Facilitating interpersonal, intra/interdepartmental relationships.* Sharing knowledge of processes and systems freely.* Continually evaluates quality of patient care and care environment to ensure optimal patient outcomes.* Documents according to Nursing Department policy reviewed/approved by CNII or Preceptor.* Works with CN II to develop individualized plan of care based upon assessed needs, input from patient/family, and other disciplines.* Implements plan of care according to unit standard timeframe and CNII/Preceptor guidance.* Reviews lab values and clinical data, incorporates them into the plan of care and reports per policy and standard of care.* Begins to identify emergency situations and respond with appropriate interventions.* Utilizes basic principles of effective communication to meet the needs of the patient/family.* Evaluates patient/family response to nursing interventions and alters the plan of care, as necessary.* Attends and contributes to patient care conferences.* With assistance from CN II or Preceptor, identifies discharge needs.Collaboration:* Recognizes the need to establish working relationships with peers and other health care providers.* Works with health care team to identify methods for meeting patients' needs.* Recognizes need for referral and confers with appropriate staff to initiate process.* Supports, philosophy, values, and mission of the organization and changes that are necessary for improvement of patient care.* Identifies patient care/unit issues and initiates problem-solving techniques following appropriate channels.* Collaborates with physicians to assure availability of self for rounds and access to current information on status of groups of patients.Personal/Professional:* Participates in growth and improvement in health care within the unit.* Participates in committees and task forces to improve patient care and organization.* Is aware that the organization supports nursing research.* Participates in educational activities for personal and professional growth.* Attends unit educational programs and in-services.Leadership:* Keeps supervisory personnel informed of significant changes in patient status.* Evaluates own care and begins to evaluate care given by others. Seeks assistance as needed from CN II to give feedback to other caregivers.* Maintains accountability for own care and care delegated to others. Adheres to ethical and legal standards.* Seeks assistance from appropriate resources, when needed.Patient/Family Education:* Begins to assess learning needs of the non-complex patient and utilize appropriate resources to provide patient education.* Explains treatment and procedures to patient.* Utilizes available patient educational tools.* Seeks expertise of other health care team members.* Maintains universal precautions on all patients.Performs other duties, as assigned including, but not limited to the following:* Assists patients with activities of daily living.* Assists patients' personal hygiene activities such as bathing, dressing, hair care, oral hygiene, back and skin care, perineal care and indwelling urinary care.* Assists patients in using bedpan, urinal, and/or commode and records output on patient flow sheet.* Feeds patients on oral and tube feedings.* Observes patients and keeps responsible licensed staff informed of general condition and/or change in their status.
